What we do?
At CASA we provide personalised support to people in their own homes, helping them to live independent and full filling lives. Working in the community provides you with a varied role, working with different groups of people including the elderly, children, people with mental health, learning disabilities, physical disabilities, complex needs or in need of palliative care. This enables every day to be different.
What you would do?
As a Reablement Care Assistant, you will spend your time out in the community visiting our service users assisting them to gain independence following on from a hospital discharge. We will be responsible for assisting service users to meet their identified goals within a 3 week time frame and working collaboratively with other healthcare professionals to ensure a safe and successful discharge. The type of care and support that we offer varies depending on each customer’s needs.
All our customer’s have a care plan in place for you to adhere to, however, please see below a list of potential tasks that you may be expected to carry out:
Who we are looking for?
Our ideal candidate would have experience working within a caring role, have an enthusiastic attitude and an excellent communicator. Driving licence and access to a car is preferred.
